1. Member understands that Medical Discount Solutions of Oklahoma (MDSO) is not
an insurance plan or policy. No payments to medical providers or members will be
made by MDSO. Actual savings may vary, depending upon location and specific services/products
rendered.
2. Member understands that they will be charged a non-refundable $25.00 USD registration
fee.
3. MDSO provides savings to its members on healthcare services through various healthcare
providers. In order to access these providers at the related discounts, member(s)
must pay the provider at the time services and/or products are received unless other
arrangements are made with said provider. If a member has difficulty using the program,

4. Neither MDSO, nor any of its affiliates shall be liable for any payment to a
provider accessed under the MDSO program. Neither MDSO, its affiliates, nor any
associated provider are guarantors or underwriters of the responsibility or liability
of Member for Member's or Member's dependant's medical care or any other goods or
services provided to Member or Member's dependents. A directory of participating
providers and corresponding discounts is provided to members on www.MDSOOK.com and
is subject to change without notice.
5. Participating Healthcare Providers are independent Providers and neither MDSO,
nor its affiliates are responsible for healthcare provided or the omission of the
provision of health care by any provider. MDSO does not practice medicine or in
any manner interfere with or participate in the provider-patient relationship. All
health care decisions are between the patient and provider. The selection of a provider
is the obligation and decision of the patient and is not based upon the credentialing
or any recommendation by MDSO.
6. Membership payments will be debited from your account between 1st and 5th of
each month unless alternative arrangements have been made. Member waives the right
to charge back on credit card and bank drafts accounts. If you choose to cancel
your program, it is your responsibility to return your membership cards to MDSO
along with written notice of cancellation. Terminated Memberships will be assessed
a $75 reenrollment fee if the Member wishes to renew the terminated agreement.
7. Returned checks, insufficient fund notices on bank drafts or denial by member's
credit card company for payment of the monthly membership fee is deemed evidence
of non-payment by a member and may cause termination of membership at the discretion
of MDSO and will result in a NSF service charge of $20.00 being assessed. Members
failing to fulfill their financial obligation to MDSO may be subject to attorney's
fees and reasonable costs of collection.
8. MDSO is not a hospital Emergency Room. If you have a true emergency, call "911."
9. MDSO reserves the right to terminate any membership for failure to pay a healthcare
provider accessed under the MDSO program under the terms provided.
10. There is a 30 day waiting period for new members to utilize the Beech Street
network, however new members will have immediate access to American Dental and Exclusive
networks.
